We are seeking a dedicated and experienced Advanced Clinical Pharmacist to join our team at the South West Yorkshire Partnership NHS Trust. As a key member of our pharmacy team, you will be responsible for providing high-quality patient-focused pharmaceutical care to designated teams. This is an exciting opportunity to contribute to the delivery of integrated, person-centered pharmacy care and services.

About the Role

As an Advanced Clinical Pharmacist, you will provide clinical pharmacy services to allocated teams, ensuring that patients receive safe and effective medication management. You will work collaboratively with healthcare professionals to deliver patient care, contributing to multidisciplinary team meetings and providing education to patients and professionals. Your expertise will also include medicines reconciliation targets for inpatient areas, screening for service users admitted to intensive home-based treatment services, and participating in in-house training for pharmacy staff and healthcare professionals.

Requirements

To be successful in this role, you will require current registration with the General Pharmaceutical Council (GPhC), membership of the Royal Pharmaceutical Society (RPS), and substantial post-registration experience as a pharmacist. Experience in mental health pharmacy, hospital pharmacy, and provision of medicines information/advice/education to service users, carers, and healthcare professionals is highly desirable. A strong understanding of audit planning and delivery, as well as computer literacy, including pharmacy systems and Microsoft Office, is essential.

Salary and Benefits

The estimated salary for this position is £63,000 - £85,000 per annum, depending on experience. The contract is a one-year fixed-term appointment, and you will have access to excellent benefits, including a contributory pension scheme and generous annual leave entitlement.
