Role Summary The Clinical Nurse Manager will lead the operational management of the Virtual Ward and Acute Hospital at Home team to ensure that:- High standards of care delivery are achieved and maintained.- Such services are delivered in an efficient and cost-effective way and within allocated resources.
The Clinical Nurse Manager will be based in the acute Hospital in Eastern.
To meet the needs of the service, the post holder may be required to work in other areas as appropriate as directed by the line manager.
The post holder will work as part of a team, and will work closely with the Clinical Nurse Manager for AHAH in Northern and will need a good working relationship with the Acute Medicine CNMs in Eastern.
They will also need to work with community colleagues.

More detail about the role An exciting opportunity has arisen within Acute Medicine for a dedicated, enthusiastic and experienced Registered Nurse to lead our integrated Acute Hospital at Home team, working closely with the Lead Nurse and the Band 7 Clinical Nurse Manager in our Northern services.
Acute Hospital at Home is a safe and efficient alternative to NHS bedded care.
It is a Virtual Ward, which supports patients who would otherwise be in hospital to receive the acute care and treatment they need in their own home.
This includes either preventing avoidable admissions into hospital or supporting early discharge out of hospital.
The post holder will: Function as a professional role model and clinical leader, demonstrating both clinical and managerial competence.
Be highly visible and accessible in clinical areas to facilitate communication with staff and patients, observe clinical practice and ensure standards are maintained.
Promote and support innovative practice and ensure relevant research findings are incorporated into nursing practice.
This will include creating and developing new pathways, and regularly reviewing those already in use.
They will work to improve and maintain the AHAH service based on best practice and current national guidelines.
They will also manage the Acute Medicine Ambulatory Care service and the Acute Medicine Care-Coordinators, ensuring the service runs smoothly and is utilised appropriately.
The delivery of digitally enabled care (virtual care) including virtual wards is a national priority embedded within current health and social care policy.
This post, as part of a multi-disciplinary team, provides an opportunity to drive and transform the system by embedding the use of digital technology to release pressure on acute care and improve patient outcomes.
The successful candidate must have excellent communication skills and work well as part of a multi-disciplinary team.
You must have the ability to work under pressure and be interested in personal development of leadership skills.
A priority is service development and improvement as part of continuing expansion of the Virtual Ward.
As we move towards a new era of integrated care and working collaboratively across organisational boundaries, the need for dynamic, focused teamwork is vital.
Working Pattern: Full time, 4 day shifts a week.
This will usually be weekdays, but may include weekends and bank holidays as the services requires.
