About the Role:


We're seeking a highly skilled and motivated Specialist Wellbeing Nurse to join our team, responsible for running and organizing an AKI outpatient clinic. The successful candidate will work closely with a multidisciplinary team to provide clinical guidance and support to patients identified as needing further advice and management following discharge. This is a fantastic opportunity for a nurse to develop their skills and knowledge in a specialist area and make a real difference in the lives of our patients.



Responsibilities:



  • Run and organize an AKI outpatient clinic for patients identified as needing further advice and management following discharge
  • Work closely with a multidisciplinary team to provide clinical guidance and support to patients
  • Participate in clinical audit and quality improvement programs
  • Evaluate research and present findings to a multidisciplinary team
  • Contribute to the development of clinical guidelines and policies


Requirements:


The ideal candidate will have a strong background in nursing, excellent communication and interpersonal skills, and the ability to work effectively in a fast-paced environment. A car would be desirable, but not essential, as the role involves traveling to clinics in the Exeter and East Devon region.
