Job summary To act asan autonomous practitioner who is able to work independently and in conjunctionwith other members of the clinical team.
The post holder will be an experiencedcaring nurse with a specialty in diabetes who can demonstrate developed clinical skills and be able todemonstrate critical thinking whilst working within their professionalboundaries.
They will work the Surgery clinical team, consisting of GPs,ANPs, Practice Nurses and Health Care Assistants to deliver excellent primarycare services within the practice.This rolewill demonstrate safe, competent clinical decision-making and deliver expertcare, including assessment, diagnostic skills, treatment and will communicateat all levels across boundaries.
The post holder will work collaboratively withthe multidisciplinary team to meet the needs of patients, supporting the deliveryof policy and procedures, and providing nurse leadership as required Main duties of the job The position is Part-Time 30hrs per week.
The role is structuredwith an organised work flow, with set appointment numbers and the Postholdershould expect a mixof same-day and pre-booked diabetic review encounters.

Assessment,diagnosis, treatment, prescribing, referral and discharge as appropriate anddeliver key elements of Surgery initiatives.
Assess,diagnose, plan, implement and evaluate treatment/interventions and care forpatients presenting with an undifferentiated diagnosis.
Instigateinvasive and non-invasive diagnostic tests or investigations, interpretation of findings and reports.
Directlyadmits patients to secondary care outlets in medical need.
Referpatients to other medical specialities for assessment where indicated includingfast track referrals To takeresponsibility for making autonomous decisions in relation to patient care.
Clinicallyexamine and assess patient needs from a physiological and psychologicalperspective, and plan clinical care accordingly.
Assess,diagnose, plan, implement and evaluate interventions/treatments for patientswith complex needs.
Break badnews to patients/parents/carers and provide appropriate ongoing support to meetidentified needs Tocollect data and participate in regular Practice audit and research projects Workeffectively with individuals in other agencies to meet patients and carersneeds Recogniseand work within own competence and professional code of conduct as regulated bythe NMC Produceaccurate, contemporaneous and complete records of patient consultation,consistent with legislation, policies and procedures on the Surgery clinicalsystem Delivercare according to local and national guidelines and evidence-based carePeople Management: Provideexpert Diabetes and nursing advice to others working within the team.
To maintain own 3 yearly registration with theNMC To maintain own continuous professionaldevelopment and ensure PDP kept up-to-date in line with the requirements of thePracticeStrategic Management Work with the clinical and management team todevelop new services for patients Contribute to practice incentive schemes, diseasemanagement registers and Surgery targets set locally and nationally for patientcare.Special Requirements of the Job: An understanding, acceptance and adherence tothe need for strict confidentiality.
Competence in the use of IT systems, codingwork as completed, with the ability to learn new software.General This jobdescription covers the main duties which the post holder is expected toundertake.
