Job summary The Norfolk Community Health and Care NHS Trust Diabetes Team is primarily based in West Norfolk.
We are a growing, friendly, and supportive team who provide care for people living with diabetes in a variety of settings including clinics, patient's homes and in care settings.
We are a team of highly skilled, motivated individuals, each offering their own areas of expertise and we pride ourselves on delivering high standards of care at all times, while embracing change and innovation.
As a team we are consistently looking at ways to improve the delivery of care we provide and educational support to other health care providers.
As part of an innovation, a fantastic opportunity has arisen to work within the NCHC Diabetes Team.
We have two positions to fill at Band 6 both for week.
You will be working in either Norwich or North Norfolk alongside a Band 4 Assistant Practitioner and under the supervision of the Diabetes Specialist Nurse Lead for NCHC.
Please Specify within your application your 1st and 2nd preference for North Norfolk or Norwich.
The role does require professional and clinical experience.
Due to the nature of this condition and ever evolving service changes there is a high need for personal resilience.
These group of patients frequently have complex health needs and require high levels of emotional, educational, and professional support, however the team and wider organisation are a great support network.
Main duties of the job You will work within the Diabetes Team providing expert knowledge and skills.
You will be responsible for managing a caseload of patients with complex diabetes needs, using evidence-based practice and patient centered goals to assess, plan, implement and evaluate interventions.
The role will include health education to the service users, their families, carers', and other agencies involved in their care management and you will work to promote and encourage self-management wherever possible.
You will be required to work independently working in patient's homes across your place with access to the diabetes team remotely.
The successful candidate will hold a full UK driving licence and have access to their own vehicle as you will be required to travel independently around the county.
o To provide an expert resource for primary health care teams to provide care and support for people with Diabetes in the community setting.
Holistic review of all patients on the community insulin caseload.
Act as a link nurse for nursing and residential homes, assist with training.
Provide advice and guidance for the community nursing teams on diabetes management.
Support identified patients to become independent with insulin administration and management including new to insulins and existing patients.
To review new referrals to community insulin caseloads.
working collaboratively with other organisations.
About us The community diabetes service is a busy nurse led team, we work across west Norfolk in assessing and managing people with complex diabetes and other co-morbidities.
We offer support and education to other health care professionals and run structured education sessions for patients.
We offer the opportunity to work flexibly within the service, and have opportunities to develop and learn new skills.
The successful applicant will have experience with working with diabetes management and patients living with diabetes and will have evidence of extended learning.
Post registration qualification in diabetes is desirable.

Provision of education and support to patients who have diabetes to enable them to self-manage and reduce their risk of complications from diabetes.
Delivery of expert care and support in a variety of settings including home visits and telephone support.
To provide educational support to professionals running Diabetes clinics in general practice for them to provide patients and their carers with systematic care and support.
Deliver a specialist Diabetes Service to community care, clinics, nursing/care homes and housebound patients across the primary/secondary interface which compliments and enhances the existing community based service.
Promote and raise the profile of diabetes management within local trusts, to professionals, patients and their carers and families.
Support the improvement of standards of care for people with Diabetes in line with the National Service Framework (NSF) for Diabetes and National Institute for Health and Care Excellence (NICE).
Person Specification Qualifications Essential Registration in adult nursing with extensive post-registration experience Desirable Experience in diabetes 1st Degree or working towards Qualification in diabetes management Experience Essential Recent clinical experience in management of individuals with diabetes Desirable Recent clinical experience in management of individuals within Community Care Skills, Abilities and Knowledge Essential Excellent interpersonal skills Able to demonstrate management skills and experience Able to demonstrate examples of innovative practice Able to work effectively within teams, demonstrating facilitative leadership skills Good knowledge of relevant NHS policy, most notably NSF for Diabetes and NICE Desirable Advanced clinical assessment skills Knowledge and understanding of change management Communication Essential Good communication skills face to face and on telephone as will interact with the public and other healthcare professionals personal and people development Essential Reliable and adaptable Evidence of good interpersonal skills Ability to work within a team Personal Attributes, Behaviours Essential Able to identify with the Trust's commitment to safeguarding and promoting the welfare of children and young people/vulnerable adults.
