About This Role

We are looking for a dedicated Diabetes Assistant Professional to join our Diabetes Team at the Transformation Unit.

In this role, you will be responsible for providing care to patients living with diabetes in various settings, including clinics, homes, and care settings.

You will work under the supervision of the Diabetes Specialist Nurse to assess and review patients with diabetes, optimise treatment regimes, and provide health education to service users, their families, carers, and other agencies involved in their care management.

The ideal candidate will have a foundation degree programme or equivalent demonstrable knowledge and skills acquired, along with experience of working in health or social care settings. A full UK driving license and access to a vehicle are required.

We offer a competitive salary of £26,530 - £29,114 per annum pro rata, dependent on experience.
