Job summary Tendring Primary Care Network (PCN) has two posts available for this exciting new proactive care team service, Advanced Nurse Practitioner/Advanced Clinical Practitioner/Nurse Specialist - Proactive Care.
Main duties of the job We are open to applications from qualified Advanced Nurse Practitioners (ANP), Advanced Clinical Practitioners (ACP) or Nurse Specialists within this field or with relevant experience.Developmental roles would also be considered for the right candidate(s).
About us Our network is located on the North-East Essex Coast and comprises of 3 GP practices: Mayflower Medical Centre in Harwich, St James Surgery in Clacton, and Great Bentley Surgery.
It has an approximate patient population of 44,000 and it strives on bringing together local health care and resources to deliver high quality, evidence-based and integrated care to all their patients.
This is an exciting time to join Tendring PCN as we are one of the test sites for the national PCN Pilot Programme, delivering new ways of working and projects in line with the Fuller pilot to improve experiences for our workforce and patients, whilst aiming to sustain general practice for the future.

Person Specification Qualifications Essential You will be a qualified advanced nurse practitioner/advanced clinical practitioner or nurse specialist with strong experience within primary care and the community.
You must have excellent clinical and leadership skills, be proactive and self-motivated, but most importantly, have a passion for maintaining high standards of care and developing services with the patient at the centre of your delivery.
