Job summary Band 6,Speech and Language Therapist 15 HOURS PERWEEK We have an exciting opportunity for a Qualified Speech and LanguageTherapist to join our well-established Memory Services Teams.
We provide specialist assessment for people whoare presenting with early signs of dementia illness.
As a speech and Languagetherapist, you will complete a specialist SLT assessment to aid in thediagnosis progress and provide specialist SLT /communication interventions.
The service operation between the hours of Monday to Friday 9 till 5excluding bank holidays.
Flexible working patterns will be considered.
The team is based in the Benet building atBelmont which is in the grounds of the Belmont Abbey which is peaceful rural location,while being 2 miles from the main city of Hereford.
There is plenty of spaceprovided for free parking.
The team works between a spilt of office based workand working from home in line with the service requirements and personalpreferences.

Main duties of the job Work with patients to identifytherapeutic goals as part of the overall recovery-focused care plan, usingspecialist mental health skills.
Adhere to Trust andprofessional guidelines around the safe administration and storage of the medication.Effective and timely dischargeplanning and coordination.
Ensure timely reviews of careplans and assessments.
Manage acaseload of patients with complex mental health needs (taking on the role ofcare coordinator where relevant).
Plan andimplement patient-centred individual and/or group interventions to achievetherapeutic goals.
Monitor,evaluate, and modify treatment in order to measure progress and ensure the effectivenessof an intervention.
Apply a highlevel of understanding of the effect of mental health needs and providetraining and advice on management of the condition, healthy lifestyles andwell-being.
Provideeducation, consultation and advice to other care settings Apply in depthknowledge of any relevant health and social care Legislation and implicationson intervention.
Carry outcomplex risk assessment.
Contribute tothe Trusts and Teams clinical governance arrangements and quality agenda,including the setting and monitoring of practice standards.
Timely andaccurate reporting and escalation of incidents and safeguarding concerns.
Attend regularmultidisciplinary meetings.
Participate inundertaking professional cover/duty.
