If you are currently a support worker or health careassistant with plenty of previous experience of supporting individuals with alearning disability then you could be just the person that we are looking forto join our small, friendly, and dedicated team.
Thisis an exciting, challenging and very varied role with no two days ever being thesame.
You will be supported with supervision and guidance of the clinical team(which includes nursing, speech & language, physiotherapy, occupationaltherapy, and psychology to support assessments and implement plans of care tomeet the identified health needs of individuals.There also are good public transport links to the mainbase but it is essential you can meet the travel requirements of the role.

As part of theCommunity Learning Disability Team, deliver a nursing service which helps meetthe health needs of people with learning disabilities including those withcomplex needs.
Also, facilitate and promote access to generic medical andhealth services.Under the supervision of an allocated clinician assess,monitor and evaluate service users' health and support needs, throughdiscussions with carers/service users, direct observation, liaison with otherhealth providers, and provide intervention, health and care resources accordingto their needsThe trust offer access to training to support your on-going professional development, regular clinical and line management supervision, and annual appraisals to support you with your on-going knowledge development and career progression within this role.

Job description Job responsibilities Ensure written records are kept up to date on care notes/ Rio tofacilitate evaluation and monitoring of all patients.
Obtain required information during assessment period and planservice users' care through discussions with the supervising allocated clinician.Implement health-related interventions, train service users andtheir carers in their implementation and contribute to monitoring andevaluating their effectiveness.
After appropriate training, carry out well- being checks andobservations; to devise action points arising from the checks and ensure theirimplementation including referral to other agencies, when necessary.
Support service users and their carer in implementing healthinterventions and in monitoring their service users' health.
Facilitate service users' access to medical and healthappointments and support each service user through these appointments ensuringthat information is given to them in a way that they understand.Ensure that treatment plans are made for treatment to beeffectively implemented liaising with other carers, as required, and passingon any concerns to the supervising allocated clinician.
Respond to emergencies or sensitive issues as they arising as loneworker, using own initiative to take appropriate action.
Plan and implement health support groups; teach self-care skillsand parental skills as appropriate.
Advise and support service users and carers within limited rangeof health topics, liaise and advocate, as necessary, to ensure advice isunderstood.
Ensure that all interactions with service users are accessible andadapted to their individual needs this includes using good skills in 'plainEnglish' both spoken and written, pictorial and concrete symbols and promptswhere appropriate.
Liaise with service users, families, carers and other agencies andprofessionals, as required, by the care plan.
Use good listening skills and be able to respond appropriately andsensitively when confronted by situations of conflict or high emotion.
Maintain effective channels of communication with team members,other professionals and agencies while adhering to the Trust's confidentialitypolicy.Overcome barriers to communication while monitoring and evaluatinghealth interventions including training of service users to monitor their ownhealth.
Act as an interpreter between generic health professionals andservice users.
Know when to report concerns to Clinical lead or other healthprofessionals as needed.
Have a working knowledge of, and considering specific healthrelated issues with service users, for example, mobility, hygiene, epilepsy,healthy eating, diabetes management , dysphagia, occupation, respiratorydisorders etc.
Actively promote individual patient choice and options within thecare plan.
Participate in the monitoring and review of patients' needs withinthe CLD Team.

Person Specification Qualifications Essential Good standard of general education Level 3 qualification (, A-level, NVQ3) in Health Care.
Safety intervention trained or willing to undertake this training and update as required, Will be expected to meet the Care Certificate standards within 12 weeks of commencing employment with the Trust, I understand that I will be asked to provide original certificates to verify my qualifications at point of offer.
Experience Essential Minimum of 3 years experience working with adults with learning disabilities.
One to one working.
Personal care tasks Desirable Working within the NHS or similar large public organisation.
Community working with learning disabilities.
