The S-CAMHS Substance Use team is excited to step up our prevention, early intervention and targeted intervention work with adolescents with risk of mental health and co-occurring substance use.
We are a small but growing team of passionate clinicians providing outreach services with young people at risk of multiple harms.These new practitioner posts will be at the frontline of providing psycho-social assessments, case-holding and carrying out interventions to prevent, reduce and minimise harm.
Our team is proud to work closely with partner agencies to ensure that the support we offer is well integrated with theirs.All applicants will need to be registered Nurses, Social Workers or Occupational Therapists; those not registered in these professions should not apply as they will not be interviewed.
Main duties of the job This role will involve close liaison with partner agencies, including health and social workers, education and third sector agencies, to promote the service and support timely referrals into the team via the S-CAMHS Single Point of Contact.Successful applicants will support screening of appropriate referrals and carry out prompt psycho-social assessments of need covering areas including mental health, substance use, adverse childhood experiences and trauma, as well as social need.
They will make decisions regarding ongoing care in conjunction with senior colleagues in the team and carry out interventions as agreed to address mental health and (risk of) substance use.As part of case-holding, they will also work closely with partner agencies to ensure that plans of care are integrated and that social needs are addressed by the appropriate agency.The post holder will be expected to work flexibly as part of the team to ensure excellent and accessible delivery of service provision and to ensure continuity.
Excellent opportunities will be available for training and professional development and the post holder will receive managerial and clinical supervision from senior colleagues.Although desirable, experience in S-CAMH services is not a requirement, and we would encourage interest from a range of candidates with relevant experience.

Five community hospitals: Amman Valley Hospital and Llandovery Hospital in Carmarthenshire, Tregaron Hospital in Ceredigion and Tenby and South Pembrokeshire Hospital Health and Social Care Resource Centre in Pembrokeshire.
Two integrated care centres (Aberaeron and Cardigan, Ceredigion).
Community facilities, including: 48 General Practices (GP surgeries), 49 Dental Practices, 98 Community Pharmacies (chemists), 44 General Ophthalmic Practices (including eye health and low vision services), 38 sites providing mental health and learning disability services, care within your own homes.
Highly specialised and tertiary services commissioned by the Welsh Health Specialised Services Committee, a joint committee representing seven health boards across Wales.

Person Specification Experience Essential Demonstrable experience of utilising a range of therapeutic skills at post qualifying level with adults and/or children and families Experience providing holistic assessments of psychological and social needs, including Adverse Childhood Experiences Experience of safeguarding practice, working with children subject to a child protection plan, identifying and managing risk Ability to meet agreed/specified service targets Ability to manage own caseload and time Demonstrate high standards in written communication Able to write clear reports and letters to referrers Experience with routine outcome monitoring Desirable Experience of working in a multidisciplinary setting and working with different professional disciplines involved in the provision of children and young people's health, education and social care Experience of teaching and liaising with other professional groups Experience of having worked therapeutically with children and young people, with complex and traumatic early life experiences Qualification and Knowledge Essential Registered Mental Health Nurse, or Registered Nurse - Learning Disability, or equivalent qualification with NMC, OR Social Worker with Social Care Wales Registration, OR Occupational Therapist with HCPC Registration Good record of Continuing Professional Development to postgraduate diploma level Knowledge of Local and National developments in relation to Specialist CAMHS Knowledge of child protection, Children Act issues and other relevant legislation Desirable To hold a post qualifying award in child care or therapeutic practice Recognised therapeutic qualification and accreditation ( Family Therapy/ CBT/ CAT) Knowledge of and experience of working in mental health, including with children and young people with serious mental health problems Knowledge of and experience of working with clients with substance use difficulties Knowledge of Mental Health (Wales) Measure and WARRN Knowledge of legislation and national developments in respect of Children Looked after Language Skills Desirable Welsh Speaker (Level 1)
