Who are we?

We are an innovative, well-established integrated review team consisting ofOccupational Therapists, Therapy Support Workers and Social Workers. We strive to deliver the right support, at the right time in the right place as close to home as possible and can't wait to welcome a Physiotherapist into this team - recognising the valuable expertise they would bring in regard to rehabilitation and maximising individual independence.

Often following a period of illness, injury or disease progression, individuals require an increased level of care and support. As a RTTCRT, we want to support that individual during this period to recover and rehabilitate in their own homes and assess their needs ongoingly.

Main duties of the job

You would carry out a review assessment of the individual, with the aim of providing intervention, and rehabilitation to improve function, reduce risk of de-conditioning and improve the physical and psychological well-being of these individuals.

Following intervention, with support from the wider MDT in this team, you would proportionately assess and agree what long term support they require. Optimising on the individual's independent capacity to care for themselves and utilisation of specialist equipment before prescribing the least invasive package of care for the individual. Aiming to improve the lives of people who require support in the community and ensure prudent utilisation of our scarce caring resource.

As a versatile postgraduate, you will need to have a strong clinical grounding in older people rehabilitation, complex manual handling, and knowledge in diverse conditions.

Successful applicants will require an excellent ability to work autonomously, managing and prioritising a diverse caseload, have proven clinical reasoning/problem solving skills and provide comprehensive and professional interventions through the review assessment process. They will possess motivational and reassurance skills and be expected to support and play a role in supervising junior Physiotherapy staff, therapy support workers and students.

The post holder will be an integral part of our integrated RTTCRT and have strong connections with the wider community Physiotherapists for professional supervision, training and integration.

You will be required to participate with the Physiotherapy respiratory on-call service which is on a rota basis (training available if you dont currently have these skills) and, in the future, based on service need, provision of a flexible 7-day service.
