Senior Community Physiotherapist

Job summary An exciting opportunity has arisen within the South Rural Community Therapy Team.
We are seeking an experienced physiotherapist with a passion for delivering effective, appropriate, and timely therapy support for patients in their own homes, ensuring that they avoid unnecessary admissions to hospital and providing goal focused rehabilitation.
The South Rural Team covers a beautiful area in Suffolk that includes Hadleigh, East Bergholt and Holbrook and will be based at Beacon House, Ipswich.
Please note a driving licence and access of a vehicle is an essential requirement of this role This is a full-time position, with hours/pattern of work to be negotiated.
Part time hours may be considered.
We are fortunate enough to be co-located with our nursing and social care colleagues and work collaboratively as part of the South Rural Integrated Neighbourhood Team.
This is an opportunity to work within a knowledgeable and experienced team, working closely with the wider MDT and other community networks.
We are looking for someone that is flexible, motivated, and willing to work within a changing environment.
We will provide support through in-service training and professional forums, Mandatory training, 1:1 and personal development meetings as well as clinical rolling skills training via the practice development facilitators.
Main duties of the job We are committed to patient centred care in the right place, at the right time and provide a comprehensive supervision and training programme with career progression opportunities within the clinical and managerial fields.
The Therapy service has a high profile within the Trust operating a 7-day service and thrives on innovation and challenge.
It has taken a lead role in many developments, which are helping to shape the future of patient pathways within the local area.
You must be able to: Demonstrate experience of working within a multi-disciplinary setting Show excellent clinical skills, providing holistic patient assessments to a range of patients.
Demonstrate positive leadership qualities within the Integrated Neighbourhood Team and the wider MDT.
Provide supervision and support to staff and students.
Promote seamless transitions between hospital and home for patients in the South Rural area.

Person Specification Qualifications Essential Diploma or Degree in Physiotherapy Health Professions Council Registration Documented evidence of Continuing Professional Development Desirable Membership of an appropriate professional body Membership of a special interest group relevant to the role.
Knowledge Essential A high level of ability to reflect on and critically appraise own practice and performance Current knowledge and skills of moving and handling dependent adults and older people A knowledge of primary and community service An in depth knowledge and understanding of the NHS Plan and NSF's relating to own and associated services and the implications for service provision.
A specialist knowledge and application of current best practice in the rehabilitation of adults with physical disabilities.
Desirable Ability to direct others towards evidence based practice Skills Essential Car owner/driver/access to a motor vehicle (role will involve travel to other sites that cannot be easily accessed by public transport means) Effective leadership skills High level of presentation and training skills.
Current professional student practice placement educator training and associated skills.
High level of skill in developing and maintaining effective working relationships with relevant stakeholders Evidence of skill relevant to the rehabilitation of adults with physical disabilities gained through theory and practical course and experience.
Planning decision making skills.
Standard computer skills including use of Microsoft and e mail Experience Essential Experience of the supervision of staff and students Experience of participating in research and audit.
Experience of working with adults with complex physical, sensory and cognitive disabilities.
Experience of working with a range of primary and community services including Health, Social Care Services and the independent Sector.
Leadership experience in a Health/Social care setting.
Proven post registration experience within a multi-disciplinary service
