Job Description: We are seeking a highly motivated and experienced Disability Care Assistant to join our team. As a Disability Care Assistant, you will work closely with individuals with learning disabilities, autism, and mental health needs, providing person-centred support and promoting their independence and well-being.
Key Responsibilities:
- Providing direct support to individuals with learning disabilities, autism, and mental health needs
- Developing and implementing person-centred care plans
- Working collaboratively with multidisciplinary teams to promote individual independence and well-being
- Maintaining accurate records and reporting any concerns or issues promptly
Requirements: To be successful in this role, you must have experience working with individuals with learning disabilities, autism, and mental health needs. You should also hold a full UK driving license and be able to demonstrate flexibility and adaptability in your work.
