About the Role

We are seeking a compassionate and dedicated Home Care Support Specialist to join our team at Acuity Care Group Limited. As a key member of our care team, you will provide high-quality support to individuals with complex care needs, promoting empowerment and enabling them to access new experiences.

Key Responsibilities
  • To provide individualised support to adults and children with complex care needs, including personal care, companionship, and daily living tasks
  • To develop and maintain positive relationships with service users, their families, and other professionals
  • To work collaboratively with colleagues to ensure a seamless delivery of care services
  • To identify and address any issues or concerns related to care plans or service delivery
Required Skills and Qualifications

To be successful in this role, you will need:

  • A strong understanding of autism, learning disabilities, and mental health conditions
  • Experience working in a hospital setting or similar environment (desirable)
  • Excellent communication and interpersonal skills
  • Ability to work flexibly and adapt to changing situations
  • Willingness to undergo additional training and development as required
