Job Title: Community Specialist Palliative Care Triage

Meadow House Hospice is seeking a dedicated and experienced Community Specialist Palliative Care Triage to join our team. As a key member of our palliative care service, you will play a crucial role in ensuring that patients receive timely and high-quality specialist care.

Job Summary

We are looking for a skilled and compassionate nurse to provide specialist palliative care to adult residents of Ealing and Hounslow CCGs. The successful candidate will work collaboratively with the Primary Care Team, acute sector, social services, and other agencies to establish a close working relationship that ensures care is coordinated and responsive to individual need.

Key Responsibilities
  1. Provide evidence-based specialist palliative care to adult residents of Ealing and Hounslow CCGs.
  2. Work collaboratively with the Primary Care Team, acute sector, social services, and other agencies to establish a close working relationship.
  3. Act as a resource and educator to the primary care team and generic palliative care providers, helping to develop and empower their practice.
  4. Manage the triage of referrals into the hospice, determining urgency of response.
  5. Lead on the role of duty nurse, responding to urgent calls to the hospice from patients known to the community team and from community teams requesting advice on patient management.
Person Specification

We are looking for a candidate with:

  • NMC registration and a degree in a related subject.
  • Post-registration qualification(s) in a speciality or relevant subject.
  • Advanced Communication Skills training.
  • Level 2 Psychology training.
  • Non-medical prescribing qualification or willingness to complete this.

Desirable qualifications include an MSc or MA in a related speciality, recognised physical assessment module at Level 7, and a leadership qualification.
