Job Title: Community Specialist Palliative Care Triage

Meadow House Hospice is seeking two dedicated Band 7 Community Specialist Palliative Care Triage and Duty Nurses to join our team. This is an exciting opportunity to be at the forefront of developing and leading on the delivery of palliative care to our patients, their families, and our partners in caring.

Main Responsibilities:
  1. Collaborate with key stakeholders to ensure timely and high-quality specialist palliative care for patients referred to our service.
  2. Work at the hospice and in the community, visiting patients to undertake assessments in their place of residence as needed.
  3. Respond to urgent calls to the hospice and provide immediate advice to stakeholders and patients and their carers.
  4. Play a crucial role in ensuring excellent triage into the hospice, helping us deliver on our strategic goals for the residents of Ealing and Hounslow.
Requirements:
  • NMC registration and a degree in a related subject.
  • Post-registration qualification(s) in a speciality or relevant subject.
  • Advanced Communication Skills training.
  • Level 2 Psychology training.
  • Non-medical prescribing qualification or willingness to complete this.
Desirable Qualifications:
  • MSc or MA in a related speciality.
  • Recognised physical assessment module at Level 7.
  • Leadership qualification.
Key Skills and Attributes:
  • Evidence of a flexible approach to working independently as well as within a multi-disciplinary team structure.
  • In-depth knowledge of the speciality and use of evidence-based practice.
  • Able to demonstrate advanced communication skills and manage complex situations.
  • Leadership, negotiation, and facilitation skills.
  • Practice development skills in line with the knowledge and skills framework and agenda for change.
