Job Summary

We are seeking a highly motivated and experienced Special Needs Care Coordinator to join our team at Outcomes First Group. This is a unique opportunity to work in a supportive environment, helping children and young people with special educational needs achieve their full potential.

This is a 37.5-hour-per-week, term-time-only position with an estimated annual salary of £32,185 based on the location in West Midlands. You will work closely with our education teams to develop and implement individualized care plans, provide therapy sessions, and communicate effectively with parents, carers, and professionals.

Your Key Responsibilities:

  • Coordinate care plans and therapy sessions to meet individual needs
  • Work collaboratively with education teams to support pupil progress
  • Develop and maintain strong relationships with parents, carers, and professionals
  • Participate in ongoing professional development opportunities
