About the Role

We are seeking a highly skilled and dedicated Community Nurse to join our team at East Coast Community Healthcare CIC. As a Community Nurse, you will play a vital role in providing high-quality patient care in the community, working closely with patients, families, and healthcare professionals to deliver person-centered care.

Key Responsibilities
  • Conduct holistic assessments for patients referred to the service, identifying their needs and developing personalized care plans.
  • Provide nursing interventions, including wound care, medication administration, and palliative care, to support patients in maintaining their health and independence.
  • Work collaboratively with multidisciplinary teams to deliver planned and unplanned services, ensuring seamless care transitions and optimal patient outcomes.
  • Develop and maintain strong relationships with patients, families, and healthcare professionals, promoting open communication and trust.
  • Participate in clinical supervision and education, staying up-to-date with best practices and evidence-based care.
  • Contribute to the development of care pathways and clinical guidelines, ensuring that care is patient-centered and effective.
Requirements
  • Professional registration with the Nursing and Midwifery Council (NMC) as a Registered Nurse (Adult).
  • BSc in Nursing Practice or equivalent.
  • Experience in community nursing, with a strong understanding of patient-centered care and community health.
  • Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare professionals.
  • Competent IT skills and ability to maintain accurate and contemporaneous patient records.
