Community Nurse

We are seeking a highly skilled and dedicated Community Nurse to join our team at East Coast Community Healthcare CIC. As a Community Nurse, you will play a vital role in providing high-quality patient care and support to individuals in their own homes, clinics, or residential settings.
Key Responsibilities- Conduct holistic assessments for patients referred to the service, identifying their needs and developing personalized care plans.
- Deliver nursing interventions, including wound care, medication administration, and palliative care, to support patients in maintaining their health and independence.
- Work collaboratively with multidisciplinary teams, including healthcare professionals, social workers, and carers, to ensure comprehensive patient care.
- Participate in clinical supervision and peer review to maintain high standards of practice and continuous professional development.
- Contribute to the development of care pathways and clinical guidelines to improve patient outcomes and service delivery.
- Act as a patient advocate, respecting patient confidentiality and promoting their rights and dignity.
- Professional registration with the Nursing and Midwifery Council (NMC) as a Registered Nurse (Adult).
- BSc in Nursing Practice or equivalent.
- Experience of working in community nursing, with a focus on patient-centered care and holistic assessment.
- Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare professionals.
- Competent IT skills and ability to maintain accurate and contemporaneous patient records.
