We are seeking a highly skilled and compassionate Community Nurse to join our team at East Coast Community Healthcare CIC. As a Community Nurse, you will play a vital role in delivering high-quality care to adults in their own homes, residential care, or clinic settings.
Key Responsibilities- Conduct holistic assessments for patients referred to the service, identifying their needs and developing personalized care plans.
- Deliver nursing interventions, including wound care, medication administration, and palliative care, to support patients in maintaining their health and independence.
- Work collaboratively with multidisciplinary teams to provide comprehensive care to patients, including those with complex needs.
- Participate in clinical supervision and contribute to the development of clinical pathways and outcomes for patients requiring proactive and reactive services.
- Take responsibility for maintaining accurate and contemporaneous patient records, adhering to ECCH policies and Nursing and Midwifery Council standards.
- Professional Registration with the Nursing and Midwifery Council (NMC) as a Registered Nurse (Adult).
- BSc in Nursing Practice or equivalent.
- Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare professionals.
- Competent IT skills and ability to maintain accurate records.
- Ability to work unsupervised and demonstrate clinical knowledge and competence within the scope of clinical practice.
