Community Nurse

We are seeking a skilled and compassionate Community Nurse to join our team at East Coast Community Healthcare CIC. As a Community Nurse, you will be responsible for providing high-quality nursing care to patients in their own homes, clinics, or residential settings.
Main Responsibilities:
- Conduct holistic assessments for patients referred to the service, including environmental, moving and handling assessments, pressure area risk, and nutritional needs.
- Develop and implement individualized care plans to meet the unique needs of each patient.
- Provide nursing interventions, such as wound care, medication administration, and palliative care.
- Work collaboratively with multidisciplinary teams to ensure seamless care delivery.
- Participate in clinical supervision and education to maintain and enhance clinical skills.
Requirements:
- Professional registration with the Nursing and Midwifery Council (NMC) as a Registered Nurse (Adult).
- BSc in Nursing Practice or equivalent.
- Excellent communication and interpersonal skills.
- Ability to work unsupervised and demonstrate clinical knowledge and competence within the scope of clinical practice.
- Previous experience in healthcare, preferably in community nursing.
