Job Title: Community Nurse

We are seeking a skilled and compassionate Community Nurse to join our team at NHS. As a Community Nurse, you will play a vital role in providing high-quality care to patients in their own homes, residential care, or clinic settings.

Main Responsibilities:
  • Conduct holistic assessments, including environmental, moving and handling assessments, pressure area risk, and nutritional needs.
  • Administer medication via injection and provide wound care and specialist leg ulcer assessment and care.
  • Support patient self-care using health coaching techniques and contribute to the development of care pathways for patients requiring a range of healthcare needs.
  • Work collaboratively with residential care home colleagues to promote high standards of care and contribute to the evaluation of training programs to support patients and carers gain the necessary knowledge and skills to maintain independence.
Requirements:
  • Professional Registration with Nursing and Midwifery Council (NMC) Registered Nurse Level 1 (Adult)
  • BSc in Nursing Practice or equivalent
  • Evidence of continued professional development and ability to work unsupervised
  • Previous experience of working in healthcare and recent clinical experience in community nursing
