Job Title: Community Health Nurse

We are seeking a highly skilled and experienced Community Health Nurse to join our team at NHS. As a Community Health Nurse, you will be responsible for providing high-quality nursing care to patients in their own homes, promoting independence and improving health outcomes.

About the Role

This is an exciting opportunity to work in a dynamic and supportive team, providing care to patients with a range of needs. You will be responsible for assessing patients' needs, developing and implementing care plans, and providing education and support to patients and their families.

Key Responsibilities
  • Provide high-quality nursing care to patients in their own homes
  • Assess patients' needs and develop and implement care plans
  • Provide education and support to patients and their families
  • Work collaboratively with other healthcare professionals to ensure seamless care
  • Contribute to the development and implementation of care pathways and protocols
Requirements
  • Registered Nurse with current NMC registration
  • Experience of working in community nursing or a similar field
  • Strong assessment and communication skills
  • Ability to work independently and as part of a team
  • Commitment to delivering high-quality patient care
