About the Role

We are seeking a dedicated and compassionate Support Worker to join our team at Home Group. As a Positive Behaviour Support Worker, you will play a vital role in providing person-centred support to our customers, helping them to reach their full potential and live fulfilling lives.

Key Responsibilities
  • Provide individualised support to customers with complex needs, including those with moderate learning disabilities and autism.
  • Work collaboratively with colleagues and clinical teams to develop and implement person-centred support plans.
  • Monitor and promote the mental and physical health of customers, identifying any signs of deterioration and taking positive risk management to ensure their wellbeing.
  • Support customers to participate in their favourite activities and try new things, promoting independence and social inclusion.
  • Work on a rota basis, including days, evenings, weekends, and bank holidays, with the potential for sleep-ins and on-call shifts.
Requirements
  • Experience in a Complex Care setting, with a Level 2 Diploma in Adult Health & Social Care or willingness to work towards it.
  • Proven ability to work with customers with complex needs, including those with behaviours of distress.
  • Strong understanding of person-centred approaches and positive risk management.
  • Excellent communication and interpersonal skills, with the ability to work effectively with colleagues and customers.
  • Enhanced DBS check required.
