About the Role

We are seeking a skilled Health Care Support Worker to join our Acute Response Team based within Intermediate Care. As part of our Home First approach, this exciting post offers the opportunity to provide short-term, intensive rehabilitation at home for patients to enable them to return home from hospital sooner.

Key Responsibilities
  • Deliver delegated care across a range of disciplines as part of a team-based approach to delivering holistic, patient-centred care.
  • Support people to achieve independence through the delivery of an individualised plan of care as determined by the team.
  • Communicate effectively with other team members at all times across all of the integrated teams and demonstrate a flexible approach to the needs of the service across the 7 days.
Requirements
  • NVQ Level 3 in Health and Social Care or working towards with equivalent level of experience.
  • Good literacy and numerical skills.
  • Experience of working in a Health or Social care environment.

As a Health Care Support Worker with Hywel Dda University Health Board, you will have the opportunity to work alongside a multidisciplinary team, including OTs, Physiotherapists, Nurses, GPs, and other health and social care professionals. You will be supported to develop your skills and knowledge, and will have access to a range of learning opportunities to help you meet the Welsh Language requirements for this post.
