Key Responsibilities

  1. Deliver care to patients in their own homes, providing personal support and assistance with daily living tasks.
  2. Work collaboratively with healthcare teams to develop and implement individualized care plans.
  3. Provide emotional support and companionship to patients and their families, promoting a positive and supportive environment.
  4. Maintain accurate records and reports, ensuring seamless communication with other healthcare professionals.
  5. Participate in ongoing training and development, staying up-to-date with best practices and industry standards.

Requirements

  • Qualified to NVQ Level 2 in Health and Social Care or equivalent.
  • Experience in palliative care or a related field.
  • Excellent communication and interpersonal skills.
  • Ability to work independently and as part of a team.
  • Valid driving license and access to a car.
