About the Role

This is a Team Leader position at Avery Healthcare, a leading provider of luxury elderly care homes in the UK. The role involves supporting the home management team in delivering high-quality, person-centred care to residents, ensuring a safe, caring, and inclusive environment.

Main Responsibilities

As a Team Leader, you will be responsible for leading the person-centred care planning process, monitoring the well-being of residents, assisting with smooth transitions for discharges, and practicing safe systems of work across a range of tasks. Key Duties
  • Lead the person-centred care planning process by implementing, monitoring, and undertaking monthly evaluations, completing care plan audits, developing action plans, and being responsible for completing the action plans.
  • Monitor the well-being of each resident, being aware of any change impacting care and care planning, and ensuring that changing needs are met and written records are amended to reflect any variation.
  • Practice safe systems of work across a range of tasks and moving and handling of residents, by assessing risks and having due regard for personal safety and the safety of residents, visitors, and staff.
Requirements

Our ideal candidate must:

  • Hold an NVQ 3 or equivalent in care for care homes.
  • Have the ability to communicate effectively both verbally and in writing.
