We are expanding our services to continue providing quality care and support. As a key member of our team, you will play a vital role in delivering exceptional patient care and support.

Our Home First & Reablement service supports people return home after a stay in hospital, enabling them to regain their independence and confidence. You will work closely with our multidisciplinary team to deliver essential clinical and personal care, promoting the well-being of our patients.

Job Responsibilities
  1. Deliver practical, personal, and intimate care whilst maintaining high levels of hygiene and health and safety.
  2. Monitor progress and function of people within their reablement programme, contributing to ongoing assessment of their rehabilitation needs by liaising with team colleagues and recording progress.
  3. Connect people to community resources, promoting digital inclusion and the use of assisted technology.
  4. Be an active participant of your team, maintaining your own continuing professional development to ensure delivery of high-quality care and support.
Person Specification
  • GCSE level Pass or equivalent in Maths and English.
  • NVQ in a relevant topic (such as healthcare) or other relevant training.
  • Experience of working in a health or social care environment providing personal and intimate care to people.
Salary and Benefits

We offer a competitive salary range of £23,615 to £25,674 per year (pro rata for part-time), plus a comprehensive benefits package, including access to continued professional development opportunities.
