Hammersmith & Fulham Council is seeking an EHCP Co-ordinator to coordinate education, health and care casework across the 0-25 years cohort.

As an EHCP Co-ordinator, you will be responsible for coordinating effective, responsive and legally compliant delivery of casework. Your key deliverables include:

  1. Coordinating EHC needs assessments, annual reviews of EHC Plans and other EHC statutory casework, delivering quality on time.
  2. Managing a caseload of children and young people with an EHC Plan or undergoing EHC needs assessment.
  3. Main point of contact for parents or carers and young people on your caseload, including professionals supporting their needs.
  4. Providing customer care by involving young people and their parents in the EHC process, as well as timely resolution of concerns.
  5. Leading a multi-disciplinary network of professionals across Education, Health and Care, facilitating co-production meetings and discussions to ensure coordinated approach to meeting needs.
  6. Analyzing professional advice to produce high-quality EHC Plans.
  7. Implementing robust casework and pathway planning to ensure suitable and efficient use of resources.
  8. Supporting SEND disagreement dispute resolution functions, including facilitating next steps meetings, attending mediation, responding to complaints and support responses to tribunals and implementation of tribunal decisions.
  9. Maintaining effective record keeping, ensuring timely and consistent update of case management systems and case records.

This role involves working with various stakeholders, including parents, carers, young people, education professionals, healthcare professionals and social workers. You will need to have excellent communication and interpersonal skills, as well as the ability to work independently and as part of a team.
