Job Title: Support Partnership Manager

About the Role:

We are seeking a highly skilled and experienced Support Partnership Manager to join our team at FourSynergy Group. As a key member of our Support Partnerships team, you will play a vital role in ensuring that our most vulnerable tenants receive the care and support they need to thrive.

Key Responsibilities:

  • Collaborate with stakeholders to tailor support solutions, conduct quality audits, and manage risks to drive successful outcomes for our residents.
  • Work closely with care providers to maintain a safe, supportive, and person-centred environment across our properties.
  • Develop and implement effective support strategies to enhance the lives of our residents.
  • Manage and maintain accurate records of support plans, progress, and outcomes.

Requirements:

  • Experience in the care or supported housing sector, with a deep understanding of safeguarding, health and safety, and care quality standards.
  • Strong relationship management skills, particularly in working with care providers and other support services.
  • A compassionate approach, with the ability to handle crises and manage compliance with a focus on care outcomes.
  • Excellent project management skills, with an emphasis on enhancing support services.
  • A commitment to making a positive impact on vulnerable individuals and communities.
