Job Title: Office Planner
About the Role:
We are seeking a skilled Office Planner to join our team at Acuity Care Group Limited. As an Office Planner, you will be responsible for delivering high-quality care services to our clients with complex needs.
Key Responsibilities:
- Plan and coordinate care services to meet the needs of our clients
- Work closely with our care teams to ensure seamless delivery of services
- Monitor and evaluate the effectiveness of care services
- Identify opportunities for improvement and implement changes
- Collaborate with other departments to ensure alignment with organizational goals
- Develop and maintain relationships with clients, families, and other stakeholders
- Ensure compliance with regulatory requirements and organizational policies
- Contribute to the development of care plans and service delivery models
- Participate in quality improvement initiatives and contribute to the continuous improvement of care services
- Provide support and guidance to care teams as needed
- Stay up-to-date with industry developments and best practices
- Contribute to the development of policies and procedures
- Participate in training and development opportunities
- Perform other duties as assigned
Requirements:
- Relevant qualifications and experience in care services or a related field
- Strong planning and coordination skills
- Excellent communication and interpersonal skills
- Ability to work independently and as part of a team
- Strong problem-solving and analytical skills
- Ability to prioritize tasks and manage multiple projects
- Knowledge of regulatory requirements and organizational policies
- Ability to maintain confidentiality and handle sensitive information
- Flexibility to work in a fast-paced environment
- Ability to adapt to changing priorities and deadlines
- Strong attention to detail and organizational skills
- Ability to work in a hybrid working environment
- Ability to travel to various locations as needed
