Clinical Nurse Specialist for Children and Young People

About the Role
We are seeking a highly skilled Clinical Nurse Specialist to join our team, working with children and young people in need of healthcare support. This is an exciting opportunity to make a real difference in the lives of families and individuals.
The successful candidate will be responsible for assessing the health and social care needs of children and young people, developing comprehensive care plans, and working closely with other professionals to ensure seamless delivery of services. With a strong background in nursing or a related field, you will possess excellent communication and interpersonal skills, with the ability to build trust and rapport with service users and their families.
This role requires a high level of autonomy, with the ability to work independently, manage your own caseload, and prioritise tasks effectively. As part of a dynamic and supportive team, you will contribute to shaping the future of children's healthcare services in Hampshire and the Isle of Wight ICB (104).
Key Responsibilities
- Conduct comprehensive assessments of children and young people's health and social care needs
- Develop and implement individualised care plans that meet the unique needs of each child and family
- Work collaboratively with other professionals, including social workers, therapists, and educators, to ensure holistic support and coordination
- Maintain accurate records and reports, communicating effectively with colleagues and stakeholders
Essential Skills and Qualifications
- Registered Health or Social Care Professional with current registration (e.g., NMC, HCPC, SWE)
- Educated to degree level in a relevant subject or equivalent experience in health or social care
- Proven experience in assessment, care planning, and coordination
- Excellent communication, interpersonal, and organisational skills
Estimated Salary Range: £42,000 - £53,000 per annum, dependent on experience.
