Nurse Assessor for Children and Young People

As a Children and Young People Nurse Assessor, you will work across Children's Continuing Care, including children and young people who are eligible for S117 funding. Your key responsibilities will include completing assessments as part of the ICB delivery of the NHS Continuing Care framework, working collaboratively with other key professionals, and developing care packages.
This role requires a Registered Health or Social Care Professional with current registration with professional body i.e. NMC, HCPC or SWE, including AMHP's and mental health social workers or significant relevant experience (LD/MH/LDAP teams only). Educated to degree level in relevant subject or equivalent level of experience of working at a similar level in health or social care is also required.
