Job Title: Children and Young People's Community Nurse

We are seeking a highly skilled and dedicated Registered Nurse to join our Children and Young People's Community Nursing Service team at Sussex Community NHS Foundation Trust.

About the Role

The successful candidate will be responsible for delivering high-quality nursing care and support to children and young people in the community. This will involve working closely with other healthcare professionals, families, and carers to ensure the best possible outcomes for our patients.

Main Responsibilities
  • Deliver and maintain excellent nursing care and practice standards within the team.
  • Work collaboratively with the Team Lead to ensure consistent delivery of safe quality nursing care for children on the caseload.
  • Help implement and write children's clinical policies and standards, maintaining other standards and clinical policies in line with SCFT requirements.
  • Manage development and performance within the team, ensuring supervision and appraisals for junior staff are completed and feedback on performance is given at regular intervals.
  • Educate and support junior staff/peers, offering leadership surrounding continual professional development and performance of best clinical practice.
  • Ensure health records are maintained and compliant with Trust policy, keeping accurate, legible, contemporaneous records and written reports as required.
  • Assist the Team Lead, Safeguarding Team, and General Manager in coordinating child protection processes in accordance with Trust policy.
Requirements
  • RN Child / RSCN (or experience of working with children in acute / community healthcare).
  • Continued professional development, including evidence of reflective practice in service training, any courses attended, and clinical experience gained.
  • A recognised teaching and assessing qualification, 998 or Learning Through Mentorship.
  • Child physical assessment module level 3.
Experience
  • At least 2 years' experience working with young people.
  • Experience of planning, managing, and coordinating programmes of care in collaboration with other allied health, social, education, and volunteer agencies.
  • Evidence of a range of clinical competencies appropriate to the role.
  • Experience of teaching.
  • Evidence of post-registration professional development within the last three years.
  • Experience of clinical audit.
Skills
  • Ability to care for children.
  • Able to teach and educate students, families, and carers.
  • Able to liaise with professionals on all levels.
  • To understand the significance of the role that the nurse plays in ensuring the optimum health and well-being of the young person.
  • Proficient clinical assessment and care planning skills.
  • Knowledge of children's safeguarding procedures.
  • Communication and interpersonal skills with the ability to communicate and engage with staff, children, and families who may be experiencing difficult circumstances.
  • Able to present information, written and orally, in a clear and logical manner.
