We are seeking a highly skilled and compassionate Senior Health Practitioner to join our team at Aneurin Bevan University Health Board.
The successful candidate will be responsible for delivering nursing practice within socially vulnerable communities, ensuring that everyone is treated with dignity and their needs are met in a timely manner.
The focus of this role is to bridge the gap of health inequalities and provide education to the communities and professionals on social determinants within vulnerable groups.
Key Responsibilities- Support the Senior Nurse in delivering and supporting effective care to patients
- Assist with managing the healthcare service across socially vulnerable groups
- To deputise for nursing colleagues during any periods of absence
- To represent the primary and community care viewpoint at operational and strategic meetings when necessary
- Work closely with the wider multidisciplinary team to ensure safe effective delivery of healthcare, including mental health and public health services

Requirements
- Registered Mental Health/Learning Disabilities Nurse with NMC
