This is an exciting opportunity for a Mental Health Practitioner to join our team in Newport East. As a member of our Community Mental Health Team, you will work closely with service users and other healthcare professionals to provide high-quality care and support.
Key Responsibilities- Assess, plan and implement care for individuals with complex, severe and enduring mental health problems.
- Work collaboratively with the Multi-Disciplinary Team to ensure comprehensive care coordination.
