Job Summary: We are looking for a Personal Support Practitioner to provide high-quality care and support to citizens in their homes. As a key member of our team, you will work closely with citizens, families, and other professionals to deliver person-centred care.

Key Responsibilities:
  
  1. Deliver personal care tasks, such as bathing, dressing, and feeding, in a safe and respectful manner.
  2. Support citizens with social duties, including shopping, socialising, and maintaining contact with friends and family.
  3. Contribute to the development and implementation of person-centred care plans.
  4. Participate in case conferences and meetings to ensure seamless delivery of care services.
