Mental Health Specialist Practitioner

Mersey Care NHS Foundation Trust is a leading provider of healthcare services in the North West region, serving over 1.4 million people.
The successful candidate will work in our Secondary Mental Health setting, providing specialist mental health assessments to patients with moderate to severe mental health conditions.
This role aims to improve patient experience by ensuring seamless transitions between services, with the best support in place.
Main Duties- Evaluate individual patient needs, establish risk and determine appropriate courses of action, considering all possible options.
- Work collaboratively with patients to promote adherence to prescribed treatments, monitoring mental health medication and potential physical health interactions.
- Collate and organize contemporaneous information about local resources that may benefit patients referred to the service.
- Facilitate 'sign posting' to other relevant services and undertake short-term interventions for patients with moderate to severe mental health difficulties.
To evaluate outcomes, maintain accurate clinical data, and focus on goal-based outcomes with patients, using a holistic approach to their care and social circumstances.
This post requires evidence of post-registration study, continuing professional development, and experience working with individuals with mental health problems in various healthcare settings.
Requirements- Degree in Mental Health Nursing or equivalent qualification.
- Post-qualification experience working with adult mental health populations.
- Proven ability to lead change management and develop care excellence within the ward/team area.
- Demonstrated knowledge of relevant mental health legislation and concepts of mentorship, preceptorship, and supervision.
