We are seeking a highly organized and detail-oriented individual to join our team as a MDT Shared Care Coordinator. This is a pivotal role in the management of patients referred on urgent cancer suspected pathways and transferred between hospitals for their management.
Key Responsibilities- Manage the Multi-Disciplinary Team meetings, ensuring seamless communication and coordination among team members.
- Track and report Cancer waiting times, identifying areas for improvement and implementing strategies to reduce wait times.
- Collect and monitor Cancer Services and Outcomes Dataset data (COSD), providing insights to inform decision-making and drive quality improvement.
- Collect and monitor quality surveillance measures, ensuring compliance with regulatory requirements and industry standards.
- Liaise with Tertiary providers for patients that are sent between organizations, ensuring timely and effective communication and coordination.
- Attend the MDM as part of the Core Team, communicating complex information and ensuring accurate documentation of patient information.
- Monitor patients through their pathway, ensuring timely and effective progression through the care process.
- Work independently without supervision, managing workloads against agreed deadlines and using initiative to resolve problems as they arise.
- Degree or equivalent experience in a relevant field.
- Experience working with the NHS or another large organization, with a strong understanding of healthcare systems and processes.
- Excellent IT skills, including proficiency in Microsoft Office and ability to learn new systems quickly.
- Ability to work under pressure, managing multiple priorities and deadlines in a fast-paced environment.
- Strong communication and interpersonal skills, with the ability to build relationships with colleagues and stakeholders at all levels.
