MDT Shared Care Coordinator

About the Role
We are seeking a highly organized and detail-oriented MDT Shared Care Coordinator to join our Cancer Services Team. The successful candidate will be responsible for managing patient pathways, ensuring the shortest possible waiting times from referral to diagnosis to treatment.
Key Responsibilities
- Manage the Multi-Disciplinary Team meetings, ensuring that all relevant information is discussed and decisions are made in a timely manner.
- Track and report Cancer waiting times, identifying areas for improvement and implementing changes as necessary.
- Collect and monitor Cancer Services and Outcomes Dataset data (COSD), ensuring that all data is accurate and up-to-date.
- Liaise with Tertiary providers for patients that are sent between organizations, ensuring that all necessary documentation is completed and transferred.
- Monitor patients through their pathway, ensuring that they are brought back to the MDT for further discussion where necessary.
Requirements
- Degree or equivalent experience.
- Working with the NHS or another large organization.
- Excellent IT skills, including Microsoft Office and data management software.
- Ability to work independently, with minimal supervision.
