The University Hospitals Sussex NHS Foundation Trust is seeking a highly skilled and organized MDT Shared Care Coordinator to join our team. This role is critical in the management of patients referred on urgent cancer suspected pathways and transferred between hospitals for their management.
Key Responsibilities- Manage the Multi-Disciplinary Team meetings to ensure cancer patient pathways have the shortest possible waiting times from referral to diagnosis to treatment.
- Provide administrative support for Cancer MDMs by identifying patients for discussion, producing and circulating a list of cancer patients prior to the MDM, and communicating with relevant clinical departments, pathology, and radiology.
- Proactively manage the patients' pathway, from first outpatient appointment, through diagnostics, treatment planning, and treatment, ensuring that the tracking process is documented and highlighting pathway issues to the Lead MDT Coordinator and Operational teams.
