MDT Shared Care Coordinator

About the Role
We are seeking a highly organized and detail-oriented MDT Shared Care Coordinator to join our Cancer Services Team. The successful candidate will be responsible for managing the Multi-Disciplinary Team meetings, tracking and reporting Cancer waiting times, and collecting and monitoring Cancer Services and Outcomes Dataset data (COSD).
Key Responsibilities
- Manage the Multi-Disciplinary Team meetings, ensuring that all necessary information is available and that the meetings run smoothly.
- Track and report Cancer waiting times, identifying areas for improvement and implementing changes as necessary.
- Collect and monitor Cancer Services and Outcomes Dataset data (COSD), ensuring that all data is accurate and up-to-date.
- Liaise with Tertiary providers for patients that are sent between organizations, ensuring that all necessary documentation is completed and that patients receive timely treatment.
- Monitor patients through their pathway, ensuring that they are brought back to the MDT for further discussion where necessary.
Requirements
- Degree or equivalent experience.
- Working with the NHS or another large organization.
- Excellent IT skills, including Microsoft Office and data management software.
- Ability to work independently and as part of a team.
- Excellent communication and organizational skills.
