About the Role

We are seeking a highly motivated and organized individual to join our Community Clinical Team as a Dietetic Support Worker. The successful candidate will provide administrative and clinical support to our team of Community Clinical Dietitians, working alongside patients in their own homes, care homes, and other community settings.

Key Responsibilities
  • Provide administrative support to the Community Clinical Dietitians, including managing patient referrals, maintaining accurate records, and coordinating patient care.
  • Support patients with nutritional care plans and feeding regimens, working closely with the Community Clinical Dietitians to ensure effective patient care.
  • Communicate effectively with patients, carers, care homes, and healthcare professionals to ensure seamless care coordination.
  • Contribute to the development of policies, resources, and guidelines relevant to the role, working closely with the Community Dietetic team.
  • Participate in audit projects to monitor and evaluate patient outcomes and service satisfaction.
Requirements
  • Level 3 education or equivalent (NVQ3, A level, diploma etc)
  • Good standard of education to include GCSE or equivalent in Maths and English
  • Experience of dealing with the general public, training, teaching, and supporting others
  • Proven administrative experience, ability to work autonomously, and proficient in the use of Internet/Intranet and email
  • Knowledge of Microsoft Office software and ability to communicate effectively with patients, carers, care homes, and all grades of staff
