We are seeking a highly motivated and organized Dietetic Support Worker to join our Community Clinical Dietitians team in Torbay and South Devon. As a Dietetic Support Worker, you will play a vital role in supporting patients living in the community who require nutritional care and support.
Key Responsibilities- Provide administrative support to the Community Clinical Dietitians team, including managing patient referrals, maintaining accurate records, and coordinating patient care plans.
- Support patients with nutritional care plans, including monitoring weight, recording food and fluid intake, and providing education on healthy eating.
- Communicate effectively with patients, carers, care homes, and healthcare professionals to ensure seamless care coordination.
- Work collaboratively with the Community Clinical Dietitians team to provide high-quality patient care and support.
- Contribute to the development of policies, resources, and guidelines relevant to the role, led by the Community Dietetic team.
- Level 3 education or equivalent (NVQ3, A level, diploma etc)
- Good standard of education to include GCSE or equivalent in Maths and English
- Experience of dealing with the general public, training, teaching, and supporting others
- Proven administrative experience and ability to work autonomously
- Excellent communication skills at all levels, highly organized, and good team working skills
