We are seeking an experienced Mental Health Practitioner to join our Community Mental Health Team at Lancashire & South Cumbria NHS Foundation Trust.
Key Responsibilities- Provide nurse leadership within the multidisciplinary team and ensure the effective assessment, formulation, planning, and monitoring of care given to service users.
- Undertake mental health assessments, including risk assessments, within a multi-disciplinary assessment team.
- Refer to other clinicians within the service or refer to other services/agencies as necessary.
- Provide a fast, responsive service to assess individual needs.
Lancashire & South Cumbria NHS Foundation Trust provides a range of services, including secondary mental health care, across this area. We strive to support people in the community and work towards recovery.
Requirements- Registered Mental Health Nurse Degree/Diploma
- On-going registration with the NMC
- Specialist qualification or equivalent level in skills and development
- Able to work as part of a multidisciplinary team
- Experience of assessing, planning, and implementing and reviewing care needs
- Experience acting as a mentor/assessor of student staff
