We are seeking a highly motivated and experienced Team Leader to join our team at Praxis Care. As a Team Leader, you will play a key role in supporting our service users who have complex needs and helping them to manage their emotions.

Key Responsibilities:

  • Support service users to develop their independence skills, including tasks such as shopping, cleaning their flat, personal care, accessing the community, budgeting, and taking medication.
  • Utilize a variety of communication tools to effectively communicate with service users and staff.
  • Work collaboratively with the dedicated staff team to promote the independence of service users.
  • Contribute to the development of individualized support plans and ensure their implementation.
  • Participate in ongoing coaching and mentoring to enhance skills and knowledge within the health and social care field.

Salary Details:

£24,836 - £28,072 per annum (based on 39 hours per week). Where the role requires sleep-ins, a minimum additional payment of £45.00 per night applies.
