About the Role


As a Primary Care Mental Health Practitioner, you will be responsible for working with individuals who have mental health needs, providing them with the support and guidance they need to improve their lives. You will work closely with our primary care colleagues and other stakeholders to ensure that our patients receive the best possible care.


Your Key Responsibilities


- Review patients in primary care and assess their mental health needs
- Offer brief interventions to individuals who have mental health needs
- Work closely with social prescribers and other professionals to support patients in accessing the right services
- Develop and maintain collaborative links with the third sector to provide clinical advice and support
- Participate in pro-active mental health promotion and wellbeing initiatives

Person Specification


- Registered professional qualification (RMN/Social worker/Occupational Therapist)
- Attendance in Significant courses relevant to Mental health
- Highly developed clinical skills with an in-depth knowledge of mental health assessment and current practice
- Ability to work across services and organisations
- Ability to lone work and manage clinical caseload and prioritise as required
