We are seeking a skilled and compassionate Field Care Supervisor to join our team at City and County Healthcare Group Ltd. As a Field Care Supervisor, you will play a critical role in supporting the delivery of high-quality care support services in the community.
Key Responsibilities- Collaborate with Care Coordinators to plan and implement effective care worker rotas, ensuring the specific needs of each individual are met.
- Conduct regular reviews of individual services, including risk assessments and performance evaluations.
- Participate in multi-disciplinary reviews of service users' needs and provide guidance on care planning.
- Support the development of care workers through on-site observations, evaluations, and coaching.
- Experience in carrying out initial risk and needs assessments for service users.
- Ability to work in partnership with service users, families, carers, and healthcare professionals to deliver holistic care packages.
- Strong communication and interpersonal skills, with the ability to build effective relationships with stakeholders.
