About the Role

We are seeking an experienced and skilled nurse practitioner to join our team as a Frailty Practitioner. This is an exciting opportunity to work in a dynamic and innovative environment, providing high-quality care to our elderly patients.

Key Responsibilities

  • Provide specialist and advanced clinical skills support to medical staff and the nursing team.
  • Conduct thorough patient histories and clinical assessments.
  • Request and interpret investigations, and make differential diagnoses.
  • Develop and implement treatment plans, including prescribing where necessary.
  • Work autonomously to assess individual patient needs and provide holistic, evidence-based care.
  • Co-ordinate clinical and support services to maintain or improve the service.
  • Role model compassionate and inclusive leadership to shape the creation of a collective leadership culture within the trust.

Person Specification

Essential

  • 1st level registered nurse/AHP with substantial experience.
  • Registered Mentor.
  • Degree level study to Masters level or equivalent experience.
  • Advanced clinical skills qualification at level 7.
  • Non-medical prescribing qualification.
  • Demonstrate thorough understanding of expanded scope of professional practice issues.
  • Evidence of continuing academic and professional development - ENB998/730 or equivalent.

Desirable

  • ALS/ILS.
  • IRMER course.
