About the Role
We are seeking an experienced and skilled nurse practitioner to join our team at Northumbria Healthcare NHS Foundation Trust. As an Elderly Care Specialist, you will play a vital role in providing high-quality care to our elderly patients across various settings.
Key Responsibilities
- Conduct thorough patient assessments and develop individualized care plans.
- Work collaboratively with medical staff and other healthcare professionals to ensure seamless care delivery.
- Provide education and support to patients and their families to promote self-management and independence.
- Participate in quality improvement initiatives to enhance patient outcomes and experiences.
- Contribute to the development and implementation of clinical guidelines and protocols.
Requirements
- 1st level registered nurse/AHP with substantial experience in elderly care.
- Registered Mentor with a strong understanding of expanded scope of professional practice issues.
- Degree level study to Masters level or equivalent experience.
- Advanced clinical skills qualification at level 7.
- Non-medical prescribing qualification.
